Overview
It should be emphasized that immediate implant placement and restoration in the mandibular posterior region and maxillary esthetic zone are still critical and challenging. This is mainly caused by the limited hard tissue volume, unfavorable biotype of soft tissue, inadequate implant position, poor primary stability, gap between the implant and socket etc., which may lead to difficulty in achieving implant primary stability, and even higher implant failure rate.

Stringent case selection, comprehensive assessment, and proper treatment planning prior to surgery are ways to reduce surgical accidents and complications, such as inferior alveolar nerve, lingual plate perforation, labial bone perforation in the mandibular posterior region and maxillary esthetic zone.  These assessments allow the clinician and patient to consider treatment options, for example, guided bone regeneration, soft tissue grafting, or using a delayed implant protocol, if the site exhibits a high risk of labial plate perforation for immediate implant placement.
